Cancellation Policy/Deposit: We acknowledge all of our clients' busy schedules and time constraints, so we do our best to accommodate everyone fairly, but if we do not receive more than 24 hours' notice of cancellation, you will be billed for the full price of your service. If we receive the cancellation on time, you will be automatically refunded.
Insurance Out of Network: Your reimbursement rate is determined by your particular plan, which may involve a contracted rate payment that is applied to your deductible. Your insurance companies require that we not only verify your benefits, but your ID. We will need a driver's license or other form of photo ID, especially if you will be using your health insurance. Please verify authorization for services prior to your session and bring the authorization number with you to your first session, as well as your driver's license and insurance card.
Couples: Make sure that BOTH of you fill out our portal forms with your personal information, insurance, etc. We cannot see anyone who has not filled out our consent forms and added their insurance information if they are using insurance.
